Indications
- Symptomatic treatment of mild to moderately severe Alzheimer's dementia
- Symptomatic treatment of mild to moderately severe dementia in patients with Idiopathic Parkinson's disease
Pharmacology
- Carbamate derivative
- Structurally related to physostigmine
- Binds reversibly with and inactivates cholinesterase
- Prevents hydrolysis of acetylcholine
- Increases concentration of acetylcholine at cholinergic synapses
- Relatively specific for brain acetylcholinesterase and butyrylcholinesterase
Dosage
- Initial dose: Rivastigmine 1.5 mg twice a day
- Dose titration based on good tolerability
- Effective dose is 3 mg to 6 mg twice a day
- Maximum daily dose is 6 mg twice a day
- Different dosage for different stages of Alzheimer's Disease and Parkinson's Disease Dementia
- Retitration for treatment interruption longer than 3 days
Administration
Twice a day, with morning and evening meals
Interaction
- Exaggerates effects of succinylcholine-type muscle relaxants
- Caution with anaesthetic agents
- No concomitant use with other cholinomimetic substances
- No pharmacokinetic interaction with digoxin, warfarin, diazepam or fluoxetine
- No untoward effects on cardiac conduction following concomitant administration of digoxin
Contraindications
- Hypersensitivity to the active substance or other carbamate derivatives
Side Effects
- Gastrointestinal adverse reactions including nausea and vomiting
- Female patients more susceptible than male patients to gastrointestinal adverse reactions and weight loss
Pregnancy & Lactation
- Should not be used during pregnancy unless clearly necessary
- Excreted into milk in animals
- Not known if excreted into human milk
- Women on rivastigmine should not breast-feed
Precautions & Warnings
- Patients with sick sinus syndrome or conduction defects, respiratory diseases
- Cholinergic stimulation may increase gastric acid secretion
- May exacerbate urinary obstruction and seizures
- Monitor body weight
- Asthma or obstructive pulmonary disease
- May worsen extrapyramidal symptoms
- Lactation
Use in Special Populations
- Increased exposure in moderate renal and mild to moderate hepatic impairment
- Not recommended for use in children
Overdose Effects
- Most cases of accidental overdose have not been associated with clinical signs or symptoms
- Symptoms may include nausea, vomiting, diarrhea, hypertension, and hallucinations
- Vagotonic effect on heart rate
- Bradycardia and/or syncope may occur
- Recommendations for asymptomatic overdose management
- Use of antiemetics and symptomatic treatment for other adverse reactions
- Use of atropine in massive overdose
- Scopolamine not recommended as an antidote
