Indications
- Prophylaxis of angina pectoris
Pharmacology
- Causes relaxation of vascular smooth muscle
- Produces a vasodilator effect on peripheral arteries and veins
- Reduces left ventricular end-diastolic pressure
- Reduces systemic vascular resistance and arterial pressure
Dosage & Administration
- Dosage should be adjusted according to individual patient response
- One 2.6 mg sustained release tablet in morning and evening
- Take on an empty stomach
Interaction
- May increase antihypertensive properties of vasodilators
- May increase antihypertensive properties of calcium antagonists
- May increase antihypertensive properties of beta-adrenergic blockers
- Concomitant use with tricyclic antidepressants and alcohol may cause high blood pressure
- Concomitant use with phosphodiesterase type 5 (PDE5) inhibitors can cause fall in blood pressure
- Aspirin decreases clearance and enhances hemodynamic effects
- May reduce pharmacologic effects of heparin
- Increases bioavailability of dihydroergotamine
Contraindications
- Known hypersensitivity to nitroglycerin
- Known hypersensitivity to other organic nitrates or nitrites
- Contraindicated in acute myocardial infarction
- Contraindicated in marked anaemia
- Contraindicated in head trauma
- Contraindicated in cerebral haemorrhage
- Contraindicated in closed angle glaucoma
Side Effects
- Headache
- Tachycardia
- Postural hypotension
- Syncope
- Cyanosis
- Methaemoglobinaemia
Pregnancy & Lactation
- No evidence of safety in pregnancy and lactation
Precautions & Warnings
- Use with caution in patients predisposed to closed-angle glaucoma
- Abrupt discontinuation may lead to exacerbation of symptoms
- Discontinuation should be gradual over several days
- Requires particular attention during acute myocardial infarction
Overdose Effects
- Peripheral vasodilation with fall in blood pressure
- Reflex tachycardia
- Monitoring of cardiac function and supportive measures required
- Circulating plasma volume should be increased by fluid substitution
- Consideration of vasopressors administration in life-threatening situations
