Indications
- Schizophrenia in patients unresponsive to conventional antipsychotic drugs
- Psychosis in Parkinson's disease
Pharmacology
- Atypical antipsychotic drug
- Binds to dopamine receptors
- Has high affinity for the D4 receptor
- Antagonist at adrenergic, cholinergic, histaminergic, and serotonergic receptors
Dosage & Administration
- Initial dosage for schizophrenia: 12.5 mg once or twice on the first day
- Dosage for psychosis in Parkinson's disease: 12.5 mg at bedtime
Interactions
- May interact with myelosuppressants
- May interact with CNS active drugs or alcohol
- May be displaced by other highly protein bound drugs
- May displace protein bound drugs like warfarin and digoxin
- Interaction with cimetidine may decrease plasma levels
Contraindications
- Severe cardiac disorders
- Renal impairment (creatinine clearance less than 10 ml/minute)
- History of neutropenia or agranulocytosis
- Bone-marrow disorders
- Paralytic ileus
- Alcoholic and toxic psychoses
- History of circulatory collapse
- Uncontrolled epilepsy
- Breastfeeding
Side Effects
- Common: constipation, dizziness, drowsiness, increased watering of mouth, nausea, unusual weight gain
- Less Common: abdominal discomfort, dryness of mouth
Pregnancy & Lactation
- No adequate studies in pregnant women
- May be used in pregnancy if necessary
- Secreted in breast milk, should not be used during nursing
Precautions & Warnings
- Should not be stopped abruptly, should be tapered off
- Elderly patients are more susceptible to adverse effects
- Significant risk of agranulocytosis
- Weekly WBC testing for the first 6 months of therapy
- Use with caution in patients at risk of seizures
- May cause cognitive and/or motor impairment
- May cause anticholinergic effects
- May cause hyperglycemia
- May cause orthostatic hypotension and tachycardia
- Associated with myocarditis, pericarditis, and cardiomyopathy
Overdose Effects
- Altered states of consciousness
- Tachycardia, hypotension
- Respiratory depression or failure
- Seizures
- Fatal overdoses reported
