Indications

  • Lowering blood glucose in patients with type 2 diabetes mellitus
  • Adjunct to diet and exercise
  • Use in combination with Metformin

Description

  • Oral blood-glucose-lowering drug
  • Meglitinide class
  • Management of Type 2 diabetes mellitus
  • Rapid onset of action and rapid elimination

Pharmacology

  • Binds to specific receptors in the cell membrane
  • Closure of ATP dependent K+ channels
  • Stimulation of insulin secretion

Dosage & Administration

  • Starting dose of 0.5 mg before each meal
  • For patients with HbA1c >8%, initial dose of 1 or 2 mg before each meal
  • Taken immediately or up to 30 minutes before each meal
  • Dosage adjusted according to response
  • Maximum 16 mg daily

Interaction

  • Possible interactions with other medications
  • Inhibitors of the cytochrome P450 enzyme system
  • Inducers of the cytochrome P450 enzyme system
  • Cimetidine's effect on absorption or clearance
  • Effect on Digoxin, Theophyllin, or Warfarin
  • Effect of highly protein bound drugs
  • Increased risk of hypoglycemia with certain drug co-administration

Contraindications

  • Diabetic ketoacidosis
  • Type 1 diabetes mellitus
  • Known hypersensitivity to the drug or its inactive ingredients

  • Upper respiratory tract infections
  • Diarrhea
  • Constipation
  • Nausea and vomiting
  • Hypersensitivity reactions including rashes and urticaria

Pregnancy & Lactation

  • Safety in pregnant women not established
  • Use during pregnancy only if clearly needed
  • Excretion in human milk not known

Precautions & Warnings

  • Insulin substitution during concurrent illness and surgery
  • Risk of hypoglycemia
  • Administration with meals to lessen the risk of hypoglycemia

Overdose Effects

  • Few adverse effects with doses up to 80 mg
  • Lowering of blood glucose
  • Severe hypoglycemic reactions occur infrequently
