Indications
- Neutralising the immediate toxic effects of folic acid antagonists, e.g. methotrexate
- Calcium folinate rescue- a treatment technique using calcium folinate in conjunction with folic acid antagonists, e.g. methotrexate, to minimize systemic toxicity
- The treatment of megaloblastic anaemias due to sprue, nutritional deficiency, pregnancy, infancy, liver disease and malabsorption syndrome
Composition
- 6.249 mg Calcium Folinate Hydrate BP equivalent to 5 mg Folinic Acid
- 18.746 mg Calcium Folinate Hydrate BP equivalent to 15 mg Folinic Acid
Pharmacology
- Preparation of Calcium Folinate Hydrate which is calcium salt of folinic acid (5-formyl derivative of tetrahydrofolic acid)
- Metabolite and active form of folic acid that is involved as a cofactor for 1-carbon transfer reactions in the biosynthesis of purine and pyrimidines of nucleic acids
- Impairment of thymidylate synthesis in patients with folic acid deficiency leading to megaloblast formation and megaloblastic and macrocytic anemias
- Potent antidote for both hematopoietic and reticuloendothelial toxic effects of folic acid antagonists
- Postulated to 'rescue' normal cells from toxic effects of folic acid antagonists, in preference to tumor cells
Dosage & Administration
- Commence calcium folinate rescue within 24 hours of methotrexate administration when there is delayed excretion
- Administer Folinic Acid at a dose of 15 mg every 6 hours for 10 doses in general
- For hematologic toxicity from folic acid antagonists with less affinity for mammalian dihydrofolate reductase than methotrexate, administer 5-15 mg of Folinic Acid per day
Interaction
Folinates may counteract the antiepileptic effect of phenobarbitone, phenytoin and primidone
Contraindications
- Known hypersensitivity to calcium folinate or any other components
- Pernicious anemia or other megaloblastic anemia where vitamin B12 is deficient
- Rare hereditary problems of galactose intolerance, the lapp lactase deficiency or glucose-galactose malabsorption
Side Effects
Common side effects include mucositis, stomatitis, leukopenia and/or diarrhea
Pregnancy & Lactation
Use in pregnant women only if potential benefit justifies potential risk to the fetus; caution in nursing mothers
Precautions & Warnings
- Use with methotrexate or 5-FU under the direct supervision of a clinician experienced in the use of cancer chemotherapeutic agents
- Administer calcium folinate as soon as possible in the treatment of inadvertent overdosage of a folic acid antagonist
- Do not give simultaneously with folic acid antagonists, e.g. methotrexate, to abort clinical toxicity
- Ensure the prompt excretion of methotrexate as part of Calcium Folinate Rescue Therapy
Overdose Effects
Excessive amounts may nullify the chemotherapeutic effect of folic acid antagonists; give appropriate supportive care in cases of overdosage
