Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Blocks vasoconstrictor effects of angiotensin II
- Does not inhibit ACE (kininase II)
- Inhibits negative regulatory feedback of angiotensin II on renin secretion
- More than 12,500-fold greater affinity for AT 1 receptor than for AT 2 receptor
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily
- May be increased to 40 mg if further reduction in blood pressure is required
- No initial dosage adjustment recommended for elderly patients, moderate to marked renal impairment, or moderate to marked hepatic dysfunction
- May be administered with or without food
Interaction
- No significant drug interactions with digoxin or warfarin reported
- No significant alteration of olmesartan bioavailability with antacids co-administration
- Not metabolized by the cytochrome P450 system
- Possible interaction with NSAIDs in elderly, volume-depleted, or compromised renal function patients
- NSAIDs may attenuate the antihypertensive effect of angiotensin II receptor antagonists
Contraindications
- Contraindicated in patients hypersensitive to any component of the product
Side Effects
- Well-tolerated with an incidence of adverse reactions similar to placebo
- Withdrawal rate due to adverse reactions was 2.4%
- Common adverse reaction: dizziness
- Other adverse reactions reported in placebo-controlled trials
Pregnancy & Lactation
- Discontinue the product when pregnancy is detected
- Use during the second and third trimesters can cause injury and death to the developing fetus
- Potential excretion in human milk
- Consideration should be made whether to discontinue nursing or discontinue the drug
Precautions & Warnings
- Changes in renal function may be anticipated
- Treatment associated with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals
Overdose Effects
- No experience of overdose reported
- Most likely effects: hypotension and tachycardia
- Clinically significant hypotension requires active cardiovascular system support
