Pharmacology
- Synthetic glucocorticoid
- Decreases inflammation by inhibiting leukocyte migration

Dosage & Administration
- Oral dosages vary from 1 mg to 10 mg for adults
- Oral dosages vary from 0.03-0.20 mg/kg body weight for children
- Intravenous, intramuscular, or local injection administration
- Systemic therapy dosage in adults typically 0.05-0.20 mg/kg body weight
Interaction
- Enhanced potassium loss with diuretics and/or cardiac glycosides
- Impaired glucose tolerance with antidiabetics
- Increased risk of gastrointestinal ulceration with non-steroidal anti-inflammatory drugs
- Alteration of oral anticoagulant needs
- Decreased effectiveness with liver enzyme inducing drugs
Contraindications
- Gastric and duodenal ulcers
- Systemic and ophthalmic fungal infections
- Viral infections such as varicella and herpes genitalis
- Glaucoma
- Hypersensitivity to corticosteroids
Side Effects
- Cushing-like syndrome
- Fluid and electrolyte disturbances
- Musculo-skeletal effects including myopathy and osteoporosis
- Gastro-intestinal effects such as ulceration and haemorrhage
- Dermatological effects like skin atrophy and striae
- Central Nervous System effects including convulsions and pseudotumor cerebri
- Ophthalmic effects such as glaucoma and cataracts
- Immunosuppressive effects leading to increased susceptibility to infections
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy justified by potential benefit vs. potential risk
- Corticosteroids appear in breast milk
- Mothers taking high dosages advised not to breast-feed
Precautions & Warnings
- Use of lowest possible dose
- Caution in patients with congestive heart failure, hypertension, or renal insufficiency
- Minimize drug-induced secondary adrenocortical insufficiency
- Caution in active or latent peptic ulcers and gastrointestinal conditions
- Consideration for patients at increased risk of osteoporosis before initiating therapy
- Caution with high doses in patients with myasthenia gravis or receiving neuromuscular blocking drugs
Use in Special Populations
