Indications
- Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus
Pharmacology
- Inhibitor of DPP-4 (dipeptidyl peptidase-4) enzyme
- Increases concentrations of active incretin hormones
- Stimulates release of insulin from pancreatic beta cells
- Decreases secretion of glucagon from pancreatic alpha cells
- Lowers both basal and postprandial plasma glucose
- Decreases hepatic glucose production
- Improves insulin sensitivity
Dosage & Administration
- Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals
- Individualized dosage based on effectiveness and tolerability
- Gradual dose escalation to reduce gastrointestinal side effects
- Recommended starting dose for different patient groups
Interaction
- Potential interaction with cationic drugs

Contraindications
- Contraindicated in patients with renal impairment

Side Effects
- Most common side effects are nasopharyngitis and diarrhea
- Hypoglycemia more common in patients treated with sulfonylureas
Pregnancy & Lactation
- Caution advised during pregnancy and lactation due to lack of adequate studies in pregnant women
Precautions & Warnings
- Discontinue drug immediately in case of lactic acidosis or suspected pancreatitis
- Monitor Vitamin B12 levels annually
- Symptoms and precautions for lactic acidosis
- Instructions for alcohol consumption, other medicines, counseling, travel, and emergency
