Therapeutic Class

  • Combination Oral hypoglycemic preparations

Indications

  • Adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus

Pharmacology

  • Inhibitor of DPP-4 (dipeptidyl peptidase-4) enzyme
  • Increases concentrations of active incretin hormones
  • Stimulates release of insulin from pancreatic beta cells
  • Decreases secretion of glucagon from pancreatic alpha cells
  • Lowers both basal and postprandial plasma glucose
  • Decreases hepatic glucose production
  • Improves insulin sensitivity

Dosage & Administration

  • Maximum recommended dose of 2.5 mg Linagliptin and 1000 mg Metformin Hydrochloride twice daily with meals
  • Individualized dosage based on effectiveness and tolerability
  • Gradual dose escalation to reduce gastrointestinal side effects
  • Recommended starting dose for different patient groups

Interaction

  • Potential interaction with cationic drugs
  • Potential for metabolic acidosis with carbonic anhydrase inhibitors
  • Effect of inducers of P-glycoprotein and CYP3A4 enzymes on Linagliptin
  • Drugs affecting glycemic control

Contraindications

  • Contraindicated in patients with renal impairment
  • Contraindicated in acute or chronic metabolic acidosis
  • Contraindicated in hypersensitivity to Linagliptin or Metformin Hydrochloride

Side Effects

  • Most common side effects are nasopharyngitis and diarrhea
  • Hypoglycemia more common in patients treated with sulfonylureas

Pregnancy & Lactation

  • Caution advised during pregnancy and lactation due to lack of adequate studies in pregnant women

Precautions & Warnings

  • Discontinue drug immediately in case of lactic acidosis or suspected pancreatitis
  • Monitor Vitamin B12 levels annually
  • Symptoms and precautions for lactic acidosis
  • Instructions for alcohol consumption, other medicines, counseling, travel, and emergency
  • Risk of hypoglycemia with the combination
  • Importance of recognizing symptoms of low blood sugar

Overdose Effects

  • Usual supportive measures in case of overdose
  • Removal of Metformin Hydrochloride by dialysis
  • No dose-related clinical adverse drug reactions with Linagliptin
  • Reported hypoglycemia and lactic acidosis in Metformin Hydrochloride overdose cases
