Pharmacology

  • Calcium channel antagonist
  • Peripheral and coronary vasodilator
  • Negative inotropic activity
  • Inhibits cardiac conduction at sino-atrial and atrioventricular nodes
  • Direct dilatation of coronary arteries and arterioles
  • Reduces oxygen requirements of myocardial tissues
  • Reduces systemic pressure of cardiac 'after load'
  • Slowed electrophysiological activity through the S-A and A-V nodes
  • Reduces peripheral vascular resistance
  • High protein binding
  • Rapid absorption
  • Metabolized in the liver
  • Excreted through bile

Dosage & Administration

  • Usual dose of 60 mg thrice daily for film-coated formulation
  • Dosage may be increased to 180-300 mg/day if necessary
  • Starting dose of 30 mg four times daily for film-coated formulation
  • Divided doses may be increased at 1 to 2 days intervals
  • Higher doses up to 480 mg/day used for unstable angina
  • Starting dose of 60 mg twice daily for elderly and patients with impaired hepatic or renal function
  • Starting dose of 90 mg or 120 mg twice daily for mild to moderate hypertension sustained release formulation
  • Up to 360 mg daily may be required for hypertension sustained release formulation
  • Starting dose of 90 mg or 120 mg twice daily for angina sustained release formulation

Interaction

  • Concomitant prophylactic therapy with short- or long-acting nitrates may be administered safely during diltiazem therapy
  • Recommended caution and careful dosage titration with other drugs affecting cardiac contractility or conduction

Contraindications

  • Hypersensitivity to the drug
  • Sick sinus syndrome
  • Second or third degree AV block
  • Severe hypertension
  • Acute myocardial infarction
  • Radiographically documented pulmonary congestion

Pregnancy & Lactation

  • No adequate and controlled studies in pregnant women
  • Use during pregnancy only when potential benefits justify possible risk to the fetus
  • Distributed into milk
  • Women receiving the drug should not breastfeed their infants
  • Use alternative method of infant feeding if diltiazem therapy is considered necessary in nursing women
