Medicine Details

Indications

  • Prophylaxis of angina pectoris

Pharmacology

  • Causes relaxation of vascular smooth muscle
  • Produces a vasodilator effect on both peripheral arteries and veins
  • Reduces left ventricular end-diastolic pressure
  • Reduces systemic vascular resistance

Dosage & Administration

  • Dosage should be adjusted according to individual patient response
  • Adult dosage: one 2.6 mg sustained release tablet or capsule in morning and evening on an empty stomach

Interaction

  • May increase antihypertensive properties of vasodilators, calcium antagonists, beta-adrenergic blockers
  • Concomitant use with tricyclic antidepressants and alcohol may cause high blood pressure
  • Concomitant use with phosphodiesterase type 5 inhibitors causes fall in blood pressure
  • Aspirin decreases clearance and enhances hemodynamic effects
  • Reduces pharmacologic effects of heparin when used concomitantly
  • Increases bioavailability of dihydroergotamine

Contraindications

  • Known hypersensitivity to nitroglycerin or other organic nitrates
  • Contraindicated in acute myocardial infarction, marked anaemia, head trauma, cerebral haemorrhage, closed angle glaucoma

Side Effects

  • Headache at onset of treatment
  • Tachycardia
  • Postural hypotension and syncope
  • Cyanosis and methaemoglobinaemia

Pregnancy & Lactation

  • No evidence regarding safety in pregnancy and lactation
  • Administration to pregnant women and nursing mothers unless considered essential by physician

Precautions & Warnings

  • Use with caution in patients predisposed to closed-angle glaucoma
  • Abrupt discontinuation may lead to exacerbation of symptoms
  • During discontinuation reduce dosage gradually over several days and monitor the patient carefully
  • Particular attention needed during early days of acute myocardial infarction for hemodynamic monitoring and clinical status

Overdose Effects

  • Peripheral vasodilation with fall in blood pressure and reflex tachycardia
  • Monitoring of cardiac function and general supportive measures needed
  • Consideration of administration of vasopressors in life-threatening situations

Therapeutic Class
