Description
Used for the prophylaxis and treatment of chronic stable and vasopastic angina pectoris, hypertension, myocardial infarction, coronary artery spasm, arrhythmias, Raynaud's phenomenon, oesophageal motility disorder, and migraine.

DosageAndAdministration
- Usual dose: 60 mg thrice daily for film-coated tablet
- Dosage may be started at 30 mg four times daily and increased at 1-2 days intervals for film-coated tablet
- Higher doses up to 480 mg/day have been used for unstable angina with benefit for film-coated tablet
- Starting dose for sustained release tablet - mild to moderate hypertension: 90 mg or 120 mg twice daily (elderly once daily)
- Starting dose for sustained release tablet - angina: 90 mg or 120 mg twice daily in divided doses (elderly up to 240 mg daily)
Pharmacology
- Calcium channel antagonist
- Peripheral and coronary vasodilator
- Negative inotropic activity
- Direct dilatation of coronary arteries and arterioles to improve oxygen supply to myocardial tissues
- Reduces systemic pressure of cardiac "after load"
- Slows electrophysiological activity through the S-A and A-V nodes without affecting accessory bypass conduction or altering normal atrial action potential or intraventricular conduction
- Reduces peripheral vascular resistance as a result of vasodilatation
Interaction
Concomitant prophylactic therapy with short-or long-acting nitrates may be administered safely during diltiazem therapy. Caution and careful dosage titration recommended when diltiazem is administered concomitantly with other drugs that can affect cardiac contractility and/or conduction.

- Known hypersensitivity to the drug
- Sick sinus syndrome
- Second or third degree AV block
- Severe hypertension
- Acute myocardial infarction
- Radiographically documented pulmonary congestion

PregnancyAndLactation
No adequate and controlled studies with diltiazem in pregnant women. Should be used during pregnancy only when the potential benefits justify the possible risk to the fetus. Distributed into milk, so breastfeeding should be avoided.
