Indications

Prophylaxis and treatment of chronic stable and vasopastic angina pectoris
Management of hypertension
Effective in myocardial infarction, coronary artery spasm, arrhythmias, Raynaud's phenomenon, oesophageal motility disorder, and migraine

Pharmacology

Calcium channel antagonist
Peripheral and coronary vasodilator with negative inotropic activity
Inhibits cardiac conduction at the sino-atrial and atrioventricular nodes
Actions: Antianginal, Antiarrhythmic, Antihypertensive
Rapid absorption, high protein binding, metabolized in the liver, excreted through bile

Dosage & Administration

Usual dose of 60 mg thrice daily for film-coated formulation
Dosage may vary between individual patients
Elderly and patients with impaired hepatic or renal function should start with lower doses
Sustained release formulation dosage for mild to moderate hypertension and angina

Interaction

Concomitant prophylactic therapy with nitrates may be administered safely
Caution and careful dosage titration when diltiazem is administered with other drugs affecting cardiac contractility or conduction

Contraindications

Hypersensitivity to the drug
Sick sinus syndrome
Second or third degree AV block
Severe hypertension
Acute myocardial infarction
Radiographically documented pulmonary congestion
