Indications

Pharmacology
- Peripheral and coronary vasodilator
- Negative inotropic activity
- Inhibition of cardiac conduction at sino-atrial and atrioventricular nodes
- Direct dilatation of coronary arteries and arterioles
- Reduction of systemic pressure and cardiac 'afterload'
- Slowed electrophysiological activity through the S-A and A-V nodes
- Reduction of peripheral vascular resistance
Dosage & Administration
- Usual dose: 60 mg thrice daily (film-coated formulation)
- May vary based on patient's response
- Higher doses up to 480 mg/day used in some cases
- Recommended starting dose for elderly and patients with impaired hepatic or renal function: 60 mg twice daily
- Dose titration based on heart rate
- Mild to moderate hypertension: initially 90 mg or 120 mg twice daily (sustained release formulation)
- Up to 360 mg daily may be required for hypertension
- Contraindicated in severe hypertension
Interaction
- Concomitant prophylactic therapy with short-or long-acting nitrates
- Caution and careful dosage titration with other drugs affecting cardiac contractility/conduction
Contraindications
- Known hypersensitivity to the drug
- Sick sinus syndrome
- Second or third degree AV block
- Acute myocardial infarction
- Radiographically documented pulmonary congestion

Pregnancy & Lactation
