Indications
- Prevention of angina pectoris

Pharmacology
- Relaxation of vascular smooth muscle
- Dilatation of peripheral arteries and veins
- Reduction of left ventricular end-diastolic pressure
- Reduction of pulmonary capillary wedge pressure
- Reduction of systemic vascular resistance
- Dilation of coronary arteries
Dosage & Administration
- Oral dose of 20 mg tablet, 2 times daily
- Option for lower starting dose (10 mg)
- Oral dose of 50 mg sustained release capsule, once daily
Interaction
- Contraindicated with phosphodiesterase inhibitors
- Contraindicated with riociguat
- Additive effects with vasodilators
- Potential orthostatic hypotension with calcium channel blockers
Contraindications
- Allergy to isosorbide mononitrate
- Concomitant use with phosphodiesterase inhibitors
- Concomitant use with riociguat
Side Effects
- Headache
- Dizziness
- Hypotension
Pregnancy & Lactation
- Pregnancy Category B
- No well-controlled studies in pregnant women
- Unknown excretion in human milk
Precautions & Warnings
- Severe hypotension
- Paradoxical bradycardia
- Increased angina pectoris
- Aggravation of angina in hypertrophic cardiomyopathy
- Tolerance may occur with long-term exposure
- Potential physical dependence
- Maintenance of antianginal efficacy through prescribed dosing
- Possible daily headaches
- Light-headedness on standing
Overdose Effects
- Symptoms: Hypotension, headache, tachycardia, flushing
- Methemoglobinemia
- Treatment: Recumbent position, fluid administration, vasopressors may be required, methyline blue for methemoglobinemia
