Indications
- Treatment of adult patients with primary biliary cholangitis (PBC)
- Patients without cirrhosis
- Patients with compensated cirrhosis without evidence of portal hypertension
- In combination with ursodeoxycholic acid (UDCA) with inadequate response to UDCA
- Monotherapy in patients unable to tolerate UDCA
Pharmacology
- Mechanism of action: Obeticholic acid is an agonist for Farnesoid X Receptor (FXR)
- Key regulator of bile acid, inflammatory, fibrotic and metabolic pathways
- Decreases intracellular hepatocyte concentrations of bile acids
- Increases transport of bile acids out of the hepatocytes
- Promotes choleresis, reducing hepatic exposure to bile acids
- Pharmacodynamic markers: FGF-19 levels, cholic acid and chenodeoxycholic acid concentrations
- Cardiac Electrophysiology: Does not prolong QT interval

- Hepatic decompensation and failure in PBC patients with cirrhosis
- Severe pruritus
- Reduction in HDL-C
- Monitoring of serum lipid levels
Use in Special Populations
- Pediatric Use: Safety and effectiveness not established
- Geriatric Use: No overall differences in safety or effectiveness observed
- Hepatic Impairment: Contraindicated in patients with decompensated cirrhosis
Overdose Effects
- Dose-dependent increase in the incidence of hepatic adverse reactions
- Elevations in liver biochemical tests
- Ascites
- Jaundice
- Portal hypertension
- Supportive care administered as appropriate
