Indications
- Treatment of schizophrenia
- Maintenance treatment of schizophrenia

Pharmacology
- Affinities for serotonin and dopamine receptors
- Mechanism of action unknown
- Well absorbed after oral administration
- Not affected by food
- Not mutagenic or clastogenic

Dosage
- Initial dose of 5-10 mg once daily
- Daily dosage adjustment based on clinical status
- Maximum dose of 20 mg daily
Administration
- Can be given without regard to meals
- Consider gradual tapering of the dose while discontinuing
Interaction
- Antagonizes effects of levodopa and dopamine agonists
- Influenced by CYP1A2 and glucoronyl transferase inducers and inhibitors
- Carbamazepine may increase clearance
- Activated charcoal reduces oral bioavailability
- Caution with centrally acting drugs and alcohol
Contraindications
- Known hypersensitivity to any ingredient
- Known risk for narrow-angle glaucoma
Side Effects
- Very common: somnolence and weight gain
- Other: increased appetite, elevated glucose levels, elevated triglyceride levels, dizziness, akathisia, Parkinson's disease, dyskinesia, orthostatic hypotension, mild and transient anticholinergic effects, asthenia, edema, photosensitivity reaction
Pregnancy & Lactation
- Use in pregnancy only if potential benefits justify potential risk to the fetus
- Patients should notify physician if they become pregnant during treatment
- No reports of teratogenicity
- Patients should not breastfeed while taking the medication
Precautions & Warnings
- Use cautiously in patients with history of seizures
- Use cautiously in patients with low leukocyte and/or neutrophil counts, hypereosinophilic conditions, impaired hepatic function
- Caution with hepatotoxic medicines and centrally acting drugs
- Caution with medicines known to increase QT interval, especially in the elderly
- Caution about operating hazardous machinery
Special Populations
- Not studied in subjects under 18 years of age
- Starting dose for elderly patients: 5 mg/day
- Starting dose for patients with hepatic or renal impairment: 5 mg/day
