Indications

  • Treatment of hypertension
  • Lowering blood pressure
  • Reduction of risk for cardiovascular events
  • Can be used alone or with other antihypertensive agents

Pharmacology

  • Telmisartan blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Action is independent of the pathways for angiotensin II synthesis
  • Hydrochlorothiazide is a thiazide diuretic that affects renal tubular mechanisms of electrolyte reabsorption
  • Reduces plasma volume and increases plasma renin activity and aldosterone secretion

Dosage & Administration

  • Telmisartan monotherapy 80 mg once daily, can be titrated up to 160 mg/25 mg after 2 to 4 weeks
  • Hydrochlorothiazide 25 mg once daily, can be titrated up to 160 mg/25 mg after 2 to 4 weeks
  • May be administered with other antihypertensive drugs

Interaction

  • Caution with concomitant use of aliskiren, digoxin, lithium, other medicines for high blood pressure, NSAIDs, corticosteroids, ACE blockers, and angiotensin II receptor blockers
  • May interfere with certain laboratory tests, causing false results

Contraindications

  • Contraindicated in patients with known hypersensitivity to Telmisartan, Hydrochlorothiazide, or any other component
  • Do not co-administer with Aliskiren in patients with Diabetes

Pregnancy & Lactation

  • Pregnancy Category D
  • Use during second and third trimesters reduces fetal renal function and increases morbidity and death
  • Discontinue use when pregnancy is detected
  • Presence in human milk not established, thiazides appear in human milk

Precautions & Warnings

  • Caution in patients with kidney disease, liver disease, dehydration, untreated mineral imbalance, gout, or lupus
  • Monitor blood sugar levels in diabetic patients
  • Caution regarding body potassium levels and potassium supplements
  • May cause dizziness; avoid driving or using machinery
  • Report prolonged diarrhea or vomiting to the doctor
  • Ensure adequate fluid intake to prevent dehydration

Use in Special Populations

  • Safety and effectiveness not established in pediatric patients
  • Cautious dose selection for elderly patients
  • Initiate treatment under close medical supervision in patients with biliary obstructive disorders or hepatic insufficiency
  • Not recommended in patients with severe renal impairment

Overdose Effects

  • Manifestations may include hypertension, dizziness, tachycardia, hypokalemia, and dehydration
  • Telmisartan not removed by hemodialysis
