Indications

  • Treatment of hypertension
  • Lowering blood pressure
  • Prevention of fatal and nonfatal cardiovascular events

Pharmacology

  • Telmisartan blocks vasoconstrictor effects of angiotensin II
  • Blocks aldosterone-secreting effects
  • Does not inhibit ACE (kininase II)
  • Does not bind to other hormone receptors
  • Hydrochlorothiazide is a thiazide diuretic
  • Increases excretion of sodium salt and chloride
  • Reduces plasma volume
  • Increases plasma renin activity
  • Increases aldosterone secretion
  • Reduces potassium loss

Dosage & Administration

  • Initiate with Telmisartan monotherapy 80 mg
  • Telmisartan and Hydrochlorothiazide 80 mg/12.5 mg once daily
  • Dose titration to 160 mg/25 mg after 2 to 4 weeks
  • May be administered with other antihypertensive drugs

Interaction

  • Caution with aliskiren, digoxin, lithium
  • Caution with NSAIDs, corticosteroids, ACE blockers, angiotensin II receptor blockers
  • May interfere with certain laboratory tests

Contraindications

  • Hypersensitivity to Telmisartan or Hydrochlorothiazide
  • Contraindicated with co-administered Aliskiren in patients with Diabetes
