Indications
- Treatment of hypertension
- May be used alone or with other antihypertensive agents
- Initial therapy in patients needing multiple drugs for blood pressure control
Pharmacology
- Combination of Telmisartan and Amiodipine
- Telmisartan is an angiotensin receptor blocker (ARB)
- Telmisartan blocks vasoconstrictor and aldosterone-secreting effects
- Amiloride is a dihydropyridine calcium-channel blocker (CCB)
- Amiloride inhibits the influx of calcium ion into vascular smooth muscle
Dosage & Administration
- Usual starting dose is 40/5 mg once daily
- Patients requiring larger blood pressure reductions may start with 80/5 mg once daily
- Maximum recommended dose is 80/10 mg once daily
- Initiated if control of blood pressure unlikely with a single agent
- Dosage individualized and may be increased after 2 weeks
Interaction
- No clinically significant interaction with acetaminophen, glyburide, simvastatin, hydrochlorothiazide, warfarin, or ibuprofen
- Telmisartan not metabolized by the cytochrome P450 system
- Amiodipine safely administered with various other drugs
Contraindications
- Known hypersensitivity to the product or its components
- Pregnancy & lactation
- Biliary obstructive disorders, severe hepatic impairment, hypotension, cardiogenic shock, left ventricle outflow tract obstruction

Pregnancy & Lactation
- Pregnancy Categories C (first trimester) and D (second and third trimesters)
- Discontinue nursing or drug after considering importance to the mother
Precautions & Warnings
- Avoid fetal or neonatal exposure
- Correct any volume or salt depletion before initiating therapy
- Titrate slowly in patients with hepatic or severe renal impairment
- Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker
Use in Special Populations
- Pediatric use not established
- Not recommended for initial therapy in patients ≥75 years old or with hepatic impairment
Overdose Effects
- Most likely manifestations include hypotension, dizziness, tachycardia, and bradycardia
- If symptomatic hypotension occurs, supportive treatment should be instituted
