Indications
- Prevention and treatment of angina pectoris
- Due to coronary artery disease
- Not useful in aborting an acute anginal episode
Pharmacology
- Isosorbide mononitrate is the major active metabolite of isosorbide dinitrate (ISDN)
- Relaxation of vascular smooth muscle
- Consequent dilatation of peripheral arteries and veins
- Dilation of the veins promotes peripheral pooling of blood
- Reduces left ventricular end-diastolic pressure
- Reduces pulmonary capillary wedge pressure
- Dilatation of the coronary arteries
Dosage & Administration
- Usual oral dose is 1 tablet (20 mg) 2 times daily
- First dose in the morning and another 7 hours apart
- Maintenance doses ranging from 20 mg to 120 mg
- Dose of 10 mg (1/2 tablet) is suitable for lower dosage at the starting of treatment
Interaction
- Contraindicated with phosphodiesterase inhibitors
- Contraindicated with riociguat, a soluble guanylate cyclase stimulator
- Additive effects with other vasodilators
- Marked symptomatic orthostatic hypotension with calcium channel blockers
Contraindications
- Allergy to isosorbide mononitrate
- Contraindicated with phosphodiesterase inhibitors (drugs for erectile dysfunction)
- Contraindicated with the soluble guanylate cyclase stimulator riociguat
Side Effects
- Headache
- Feelings of dizziness
- Hypotension
Pregnancy & Lactation
- Pregnancy Category B
- Reproduction studies performed in rats and rabbits
- No evidence of harm to the fetus
- No adequate and well-controlled studies in pregnant women
- Caution should be exercised when administered to a nursing woman
Precautions & Warnings
- Severe hypotension may occur
- Caution in patients who may be volume depleted or hypotensive
- Nitrate therapy may aggravate angina caused by hypertrophic cardiomyopathy
- Daily headaches may accompany treatment
- Light-headedness on standing, especially after consuming alcohol
Overdose Effects
- Most common symptoms are hypotension, headache, tachycardia, flushing
- Methemoglobinemia may occur with massive doses
- Treatment consists of placing patients in recumbent position and administering fluids
