- Inhibits insulin release from the pancreas
- Enhances cell membrane permeability to potassium ions
- Relaxes local smooth muscles by switching off voltage-gated calcium ion channels
Dosage & Administration
- Starting oral dose of 5mg/kg body weight
- Regular blood glucose level determinations
- Usual maintenance dose of 3-8mg/kg/day
- Reduced doses may be required for renal impairment
- Higher dosages for specific conditions
Interaction
- Potentiates oral diuretics, antihypertensive agents, and anticoagulants
- Alters phenytoin levels
- Increased risk of hyperglycemia with corticosteroids or estrogen-progestogen combinations
Contraindications

- Caution in patients with cardiac failure or impaired cardiac reserve
- Cautious use in patients with cardiovascular disorders
- Monitoring of serum uric acid concentration
- Regular haematological examinations and growth assessments in children
- Careful dosage for patients with lower plasma proteins
Overdose Effects
- Hyperglycemia
- Management with insulin in severe cases
- Requirement of intravenous fluids and sympathomimetics for hypotension
Therapeutic Class
- Combination oral hypoglycemic preparations
