Pharmacology
- Calcium channel antagonist
- Peripheral and coronary vasodilator
- Negative inotropic activity
- Inhibition of cardiac conduction
- Direct dilatation of coronary arteries
- Reduction in systemic pressure of cardiac 'after load'
- Slowed electrophysiological activity
- Reduction of peripheral vascular resistance
Dosage & Administration
- Usual dose of 60 mg thrice daily for film-coated formulation
- Dose may be increased to 180-300 mg/day if necessary
- Start with 30 mg four times daily and increase dosage gradually
- Higher doses up to 480 mg/day have been used for unstable angina
- Recommended starting dose of 60 mg twice daily for elderly and patients with impaired hepatic or renal function
- Initial dose of 90 mg or 120 mg twice daily for sustained release formulation
- Dose titration may be required based on patient response
Interaction
- Concomitant prophylactic therapy with short- or long-acting nitrates may be administered safely
- Caution and careful dosage titration when co-administered with other drugs affecting cardiac contractility or conduction
Contraindications
- Known hypersensitivity to the drug
- Sick sinus syndrome
- Second or third degree AV block
- Severe hypertension
- Acute myocardial infarction
- Radiographically documented pulmonary congestion

- GIT disturbances
- Oedema
- Hepatitis

Pregnancy & Lactation
- No adequate and controlled studies in pregnant women
- Use during pregnancy only if benefits justify the possible risk to the fetus
- Distributed into milk, women receiving the drug should not breastfeed their infants
