Indications
- Sole anesthetic agent for brief procedures

Composition
- Thiopental 500 mg injection
- Thiopental 1 gm injection
- Thiopental Sodium for Injection with anhydrous Sodium Carbonate buffer
- Water for Injection
Pharmacology
- Ultrashort-acting depressant of the central nervous system
- Induces hypnosis and anesthesia
- Binds at distinct binding site associated with Cl- ionopore at the GABAA receptor
- Increases duration of time for which the Cl- ionopore is open
- Prolonged post-synaptic inhibitory effect of GABA in the thalamus
Dosage & Administration
- Administered intravenously as 2.5% w/v solution
- May be administered as a 5% w/v solution
- Peak effect of premedication reached shortly before induction
- Small "test" dose recommended for tolerance assessment
- Normal dosage for induction of anesthesia
- Different dosages for adults, children, and elderly
- Use in convulsive states with specific dosage recommendations
- Titrate dosage against patient's response to minimize risk
Interaction
- Possible increase in difficulty in producing anesthesia in patients taking alcohol or CNS depressants
- Additive action with other CNS depressants
- Increased hypotension and excitatory effects with phenothiazine antipsychotics
- Decreased requirement of Anestho with specific drugs
Contraindications
- Absence of suitable veins for intravenous administration
- Hypersensitivity to barbiturates
- Variegate porphyria or acute intermittent porphyria
- Status asthmaticus
- Severe cardiovascular disease
- Hypotension or shock
- Conditions in which the hypnotic effect may be prolonged or potentiated

- Prolonged somnolence and recovery

- Tachycardia
- Sneezing, coughing, bronchospasm

- Crosses placental barrier
- Small amounts may appear in the milk of nursing mothers
Precautions & Warnings
- Continuous attendance of anesthesia management personnel
- Support facilities for respiration and circulation required
- Administer with caution in conditions of preexisting hypotension or prolonged hypnotic effect
- Potential for habit formation
Overdose Effects
- May occur from too rapid or repeated administration
- Alarming fall in blood pressure
- Apnoea, laryngospasm, and respiratory difficulties
