Indications
- Sole anesthetic agent for brief procedures

Composition
- Thiopental 500 mg injection
- Sterile mixture of Thiopental Sodium and anhydrous Sodium Carbonate
- 1 vial of 500 mg Thiopental Sodium
- 1 ampoule of 10 ml Water for Injection
- Thiopental 1 gm injection
- Sterile mixture of Thiopental Sodium and anhydrous Sodium Carbonate
- 1 vial of 1 gm Thiopental Sodium
- 2 ampoules of 10 ml Water for Injection
Pharmacology
- Ultrashort-acting depressant of CNS
- Induces hypnosis and anesthesia
- Binds at distinct GABA receptor site
- Recovery after small dose is rapid
- Produces hypnosis within 30-40 seconds
Dosage & Administration
- Administered intravenously
- Premedication with atropine or scopolamine
- Test dose of 25-75 mg to assess tolerance
- Normal dosage for adult induction is 100-150 mg
- Dosage adjustments based on patient's response
- Specific dosages for adult, children, and elderly
- Use in convulsive states: 75-125 mg
Interaction
- Increased difficulty with alcohol or CNS depressants
- Additive action with other CNS depressants
- Increased hypotension with phenothiazine antipsychotics
- Decreased requirement with metoclopramide, aspirin, etc.
Contraindications
- Absence of suitable veins for IV administration
- Hypersensitivity to barbiturates
- Variegate porphyria or acute intermittent porphyria
- Severe cardiovascular disease
- Hypotension or shock
- Conditions prolonging or potentiating hypnotic effect
Side Effects
- Hypersensitivity reactions
- Respiratory and myocardial depression
- Cardiac arrhythmias
- Hypotension and tachycardia
- Sneezing, coughing, bronchospasm
- Anaphylactic reactions
Pregnancy & Lactation
- Pregnancy Category C
- No animal reproduction studies conducted
- Crosses placental barrier and may appear in breast milk
Precautions & Warnings
- Administered with caution in preexisting hypotension
- Facilities for support of respiration and circulation should be available
- May be habit forming
- Qualified personnel should administer
- Avoid extravasations or intra-arterial injection
Overdose Effects
- Too rapid injection may lead to alarming fall in blood pressure
- Apnoea, laryngospasm, and respiratory difficulties
- Discontinue in case of suspected overdosage
