Indications
- Anxiety disorder
- Short term relief of anxiety
- Anxiety associated with depression
- Panic disorder, with or without agoraphobia
Pharmacology
- Alprazolam is a triazole analog of the 1,4-benzodiazepine class of drugs
- Anxiolytic with hypnotic and anticonvulsive properties
- Interacts with the Gamma Aminobutyric Acid (GABA)- benzodiazepine receptor complex
- Causes dose-related CNS depressant activity
Dosage & Administration
- Treatment initiation dose of 0.25 to 0.5 mg three times daily
- Maximum dose should not exceed 4 mg/day
- Individualized dosage for maximum beneficial effect
- Lowering dose if side-effects occur
- Gradual reduction of dosage when discontinuing therapy
- Usual starting dose for elderly patients or patients with advanced liver disease
- Administration of Alprazolam 1 mg once daily in the morning
Interaction
- Aggravation of CNS-depressant action by concomitant use of other psychotropic drugs
- Aggravation by anticonvulsants, antihistaminics, alcohol, and oral contraceptives
Side Effects
- Side effects generally observed at the beginning of therapy
- Most frequent side effects are drowsiness and light-headedness
- Other side effects include depression, headache, confusion, dry mouth, constipation, etc.
Pregnancy & Lactation
- Categorized in pregnancy category D
- Avoidance in pregnancy
- Assumed excretion in breast milk
- Nursing avoidance for mothers using Alprazolam
Precautions & Warnings
- Potential for psychological and physical dependence
- Caution in patients with hepatic or renal disease, chronic pulmonary insufficiency, or sleep apnea
Use in Special Populations
- Safety and effectiveness not established in individuals below 18 years of age
Overdose Effects
- Manifestations of Xolam XR overdosage include somnolence, confusion, impaired coordination, diminished reflexes, and coma
- Employment of general supportive measures in cases of overdosage
- Immediate gastric lavage in cases of overdosage
