Description
- Used for various serious arrhythmias of the heart
- Corrects abnormal rhythms of the heart
- Broad spectrum antiarrhythmic medication
- Causes blood vessels to dilate
- May be beneficial in congestive heart failure
Dosage
- Tablet: 200 mg 3 times daily for 1 week, then reduced to 200 mg twice daily or minimum required dosage to control arrhythmia
- Injection: Starting dose of 1000 mg over the first 24 hours, with specific infusion regimen
- Tablet should be taken with meals to minimize stomach upset
Indications
- Treatment of ventricular fibrillation
- Prophylaxis of ventricular tachycardia
- Used for atrial fibrillation and atrial flutter
Mode of Action
- Delays heart's electrical system recharging
- Prolongs electrical phase during which heart's muscle cells are electrically stimulated
- Slows speed of electrical conduction
- Reduces rapidity of firing of normal generator of electrical impulses in the heart
- Slows conduction through specialized electrical pathways
- Causes blood vessels to dilate
Interaction
- May interact with b blockers and calcium channel blockers resulting in slow heart rate
- Increases blood levels of Digoxin
- Increases Flecainide, Procainamide, and Quinidine concentrations
- Interacts with tricyclic antidepressants, Warfarin, and inhibits metabolism of Dextromethorphan
Contraindications
- Patients with cardiogenic shock
- Severe sinus-node dysfunction
- Second- or third-degree atrioventricular block
- Known hypersensitivity to the drug or its components
Side Effects
- Severe lung-related reactions
- Fatal liver toxicity
- Reversible corneal microdeposits
- Peripheral neuropathy and myopathy
- Bradycardia and conduction disturbances
- Phototoxicity, skin discoloration, hypothyroidism, hyperthyroidism
- Jaundice, hepatitis, cirrhosis, nausea, vomiting
- Metallic taste, tremor, sweating, vertigo, headache
- Sleeplessness, fatigue, alopecia, raised intracranial pressure
- Ataxia, rashes, vasculitis, renal involvement, thrombocytopenia, anemia
Pregnancy & Lactation
- Not recommended during pregnancy
- May cause hypo or hyperthyroidism in infants from oral use during pregnancy
- Safe use during lactation has not been established
