Description
Amtylin is a tricyclic antidepressant with marked anticholinergic and sedative properties. It prevents the reuptake of noradrenaline and serotonin at the nerve ending.
Mode of Action
Amitriptyline inhibits the re-uptake of neurotransmitter amines, such as norepinephrine and serotonin, thereby increasing their concentration at the synaptic clefts of the brain. It affects the monoamine hypothesis in depression and may also have an analgesic effect.
Dosage & Administration
- Doses for Depression, Nocturnal Enuresis, Migraine Prophylaxis, Tension Headache
- Gradual increase in dosage as necessary
- Special doses for Children under 16 years
Interaction
- Enhanced sedative effect with alcohol and opioid analgesics
- Increased risk of ventricular arrhythmias with Moxifloxacin or Terfenadine
- Inhibition of metabolism with Disulfiram and Cimetidine
- Enhanced risk of postural hypotension with diuretics
Contraindications
- Myocardial infarction
- Arrhythmias, particularly heart block of any degree
- Mania
- Severe liver disease
- May affect the ability to drive or operate machinery initially due to sedation
Side Effects
- Anticholinergic reactions such as excessive perspiration and dry mouth
- Cardiovascular reactions including hypotension and tachycardia
- CNS and Neuromuscular effects like disturbed concentration and disorientation
- Allergic reactions like skin rash and urticaria
- Haematological effects such as bone-marrow depression
- Gastrointestinal effects including nausea and vomiting
- Endocrine effects like testicular swelling and gynaecomastia
Pregnancy & Lactation
- Pregnancy Category C
- Crosses the placenta
- Excreted into breast milk
- Potential benefit to the mother justifies the potential risk to the fetus
- Serious adverse reactions in nursing infants
- Decision should be made whether to discontinue nursing or the drug, considering the importance of the drug to the mother
Precautions & Warnings
- Caution in patients with a history of epilepsy, glaucoma, urinary retention, cardiac disease, diabetes, pregnancy, hepatic impairment, thyroid disease, increased intra-ocular pressure, and psychoses
- May aggravate mania
