Indications
- Essential hypertension
- Angina pectoris
- Vasospastic angina
Pharmacology
- Dihydropyridine calcium-channel blocker
- Long duration of action
- Influences myocardial cells
- Vascular smooth muscle
- Increase in coronary blood flow
Dosage & Administration
- Usual dose is 5 mg once daily for hypertension
- Maximum dose is 10 mg once daily
- 5 to 10 mg for angina
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers
- No interaction with Digoxin
- No interaction with Cimetidine
- Amlodipine did not significantly alter the effect of Warfarin
- Food does not alter the rate or extent of absorption
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnant women
Side Effects
- Dizziness
- Flushing
- Headache
- Hypotension
- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Increase in ischaemic chest pain
- Excessive fall in blood pressure
- Rashes
- Fever
- Abnormalities in liver function
Pregnancy & Lactation
- Pregnancy Category C
- No adequate and well-controlled studies
- Recommended that nursing be discontinued
Precautions & Warnings
- Precaution in patients with hepatic impairment
- Precaution during pregnancy and breastfeeding
Use in Special Populations
- Dosage for children with hypertension from 6 to 17 years of age
- Effect on blood pressure in patients under 6 years is not known
- Elderly patients can use it at similar doses
- Normal dosage in renal impairment
- Lower dose for patients with hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Marked systemic hypotension
- Possible shock
- Clinically significant hypotension due to amlodipine overdosage calls for active cardiovascular support
- Vasoconstrictor may be helpful
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile
- Charcoal recommended within 2 hours after administration
- Dialysis is not likely to be of benefit
