Indications
- Essential hypertension
- Angina pectoris

Pharmacology
Amlodipine is a dihydropyridine calcium-channel blocker, with a long duration of action, used for the treatment of hypertension and angina pectoris.
Dosage & Administration
- Usual dose: 5 mg once daily for hypertension
- Usual dose: 5-10 mg for angina
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions: Avoid combination with P-blockers in patients with markedly impaired cardiac left ventricular function
- Other Significant Interactions:
- No interaction with Digoxin
- No interaction with Cimetidine
- No significant alteration with Warfarin on prothrombin time
- Food does not alter absorption rate
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnant women
Side Effects
- Dizziness
- Flushing
- Headache
- Hypotension
- Peripheral edema
- Gastrointestinal disturbances
- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Ischaemic chest pain
- Excessive fall in blood pressure
- Rashes
- Fever
- Abnormalities in liver function
Pregnancy & Lactation
- Category C for pregnancy
- Discontinue nursing while Amlodipine is administered
Precautions & Warnings
- Take precaution in patients with hepatic impairment
- Take precaution during pregnancy and breastfeeding
Use in Special Populations
- Children with hypertension (6 to 17 years): Starting dose: 2.5 mg once daily, up-titrated to 5 mg once daily if needed
- Children under 6 years old: Effect on blood pressure not known
- Elderly: Equally well tolerated at similar doses as younger patients, normal dosage recommended
- Renal impairment: Normal dosage recommended, not dialysable
- Hepatic impairment:
- Dosage recommendations not established for mild to moderate impairment
- Initiate at lowest dose for severe impairment
Overdose Effects
- Symptoms: Excessive peripheral vasodilatation, reflex tachycardia, marked systemic hypotension up to shock
- Management:
- Active cardiovascular support
- Elevation of extremities
- Attention to circulating fluid volume and urine output
- Use of vasoconstrictor if no contraindication
- Intravenous calcium gluconate may be beneficial
- Gastric lavage may be worthwhile in some cases
- Use of charcoal to reduce absorption rate
