Indications

Pharmacology
- Dihydropyridine calcium-channel blocker
- Long duration of action

Dosage & Administration
- Usual dose: 5 mg once daily for hypertension
- Maximum dose: 10 mg once daily
- Usual dose: 5-10 mg for angina
- May be taken without regard to meals
Interaction
- Potentially hazardous interactions with p-blockers
- No interactions with Digoxin
- No interactions with Cimetidine
- No significant alteration of Warfarin effect
- Food does not alter absorption rate
Contraindications
- Hypersensitivity to dihydropyridine derivatives
- Pregnant women

- Increased micturition frequency
- Lethargy
- Eye pain
- Mental depression
- Paradoxical increase in ischaemic chest pain

Pregnancy & Lactation
- Pregnancy Category C
- No adequate and well-controlled studies in pregnant women
- Use during pregnancy only if potential benefit justifies potential risk to fetus
- Not known whether excreted in human milk
- Discontinue nursing while administered
Precautions & Warnings
- Precaution in patients with hepatic impairment
- Precaution during pregnancy and breastfeeding
Use in Special Populations
- Dosage for children with hypertension from 6 to 17 years of age
- Effect on blood pressure in children under 6 not known
- Equal tolerance in elderly and younger patients
- Normal dosage recommended for renal impairment
- Careful dose selection for hepatic impairment
- Caution in severe hepatic impairment
Overdose Effects
- Excessive peripheral vasodilatation
- Reflex tachycardia
- Systemic hypotension up to shock with fatal outcome
- Active cardiovascular support needed
- Vasoconstrictor may be helpful
- Intravenous calcium gluconate beneficial
- Gastric lavage may be worthwhile if overdose
- Charcoal use within 2 hours to reduce absorption rate
- Dialysis unlikely to be beneficial
