Description
- Prophylaxis and treatment of chronic stable and vasopastic angina pectoris
- Management of hypertension
- Treatment of myocardial infarction, coronary artery spasm, arrhythmias, Raynaud's phenomenon, oesophageal motility disorder, and migraine
- Calcium channel antagonist with peripheral and coronary vasodilator activity
- Reduced systemic pressure of cardiac 'after load'
- Reduced oxygen requirements of myocardial tissues
- Effective in slowing electrophysiological activity through the S-A and A-V nodes
- Reduction of peripheral vascular resistance
- Rapid absorption with high protein binding
- Metabolism in the liver with excretion through bile
Dosage & Administration
- Usual dose of 60 mg thrice daily for film-coated formulation
- Divided dose may be increased to 180-300 mg/day if necessary
- Starting dose of 60 mg twice daily for elderly and patients with impaired hepatic or renal function
- Mild to moderate hypertension: initially 90 mg or 120 mg twice daily; up to 360 mg daily may be required
- Angina: initially 90 mg or 120 mg twice daily in divided doses
- Maximum dose of 480 mg/day in some patients, especially in unstable angina
- Recommended caution and careful dosage titration for concomitant use with other drugs affecting cardiac contractility and/or conduction
Interaction
- Concomitant prophylactic therapy with short- or long-acting nitrates may be administered safely

Contraindications
- Known hypersensitivity to the drug
- Sick sinus syndrome
- Second or third degree AV block
- Severe hypertension
- Acute myocardial infarction
- Radiographically documented pulmonary congestion

Pregnancy & Lactation
- No adequate and controlled studies in pregnant women
- Use during pregnancy only when potential benefits justify possible risk to the fetus
- Distribution into breast milk
- Breastfeeding not recommended during diltiazem therapy
