Description
Mycophenolate Mofetil is an immunosuppressive agent that inhibits inosine monophosphate dehydrogenase (IMPDH) to prevent acute organ rejection in transplant patients and for the induction and maintenance therapy of lupus nephritis.
Indications
- Prophylaxis of acute organ rejection in renal, cardiac, and hepatic transplant patients
- Treatment of first or refractory organ rejection in renal transplant patients
- Induction and maintenance therapy for lupus nephritis patients
Mechanism of Action
Mycophenolate mofetil inhibits the de novo pathway of guanosine nucleotide synthesis by selectively and reversibly inhibiting IMPDH, leading to cytostatic effects on lymphocytes and decreased antibody production.
Pharmacodynamics
Mycophenolic acid, the active metabolite, inhibits the de novo synthesis of guanosine nucleotides required for DNA and RNA synthesis, particularly in B and T lymphocytes.

Precautions & Warnings
- Increased risk of viral infections and blood dyscrasias
- Can cause serious GI tract complications and increase the risk of developing malignancies
- Avoid live vaccines
- Avoid blood donation during therapy
Use in Special Populations
- Geriatric population: May be at an increased risk of certain adverse events
Overdose Effects
Expected to result in over suppression of the immune system, increased susceptibility to infections, and bone marrow suppression
