Indications
- Treatment of mild, moderate or severe heart failure
- Reduction of disease progression
- Use in patients unable to tolerate ACE inhibitor
- Use in patients not receiving certain therapies
Pharmacology
- Non-selective antagonism of beta-adrenergic receptors
- Alpha-adrenergic antagonistic activity
- Antiproliferative properties
- Scavenger of reactive free oxidant radicals
- Treatment of hypertension, angina pectoris, congestive heart failure
Dosage & Administration
- Initial dose in hypertension: 12.5 mg once daily
- Initial dose in angina pectoris: 12.5 mg twice daily
- Initial dose in heart failure: 3.125 mg twice daily
- Max dose for elderly patients: 50 mg daily
- Max dose for patients less than 85 kg body-weight: 25 mg twice daily
- Max dose for patients over 85 kg body-weight: 50 mg twice daily
Interaction
- Increased monitoring of digoxin when initiating, adjusting, or discontinuing Arilol
- 60% decrease in Cmax and AUC with rifampin pretreatment
- No alteration of in vitro plasma protein binding of warfarin with Arilol
- Potentiation of the pressor reaction with clonidine withdrawal
Contraindications
- Decompensated heart failure requiring intravenous inotropic therapy
- Bronchial asthma or related bronchospastic conditions
- Second or third-degree AV block
- Sick sinus syndrome without a permanent pacemaker
- Cardiogenic shock
- Severe bradycardia

Precautions & Warnings
- Caution in hepatic impairment
- Monitor clinical status in heart failure
- Monitor renal function
- Ensure heart failure is not deteriorating before increasing dose
