Usability Features
- Indicated for patients unable to tolerate an ACE inhibitor
- Indicated for patients not receiving digitalis, hydralazine or nitrate therapy

Indications
- Treatment of mild, moderate or severe heart failure of ischemic or cardiomyopathic origin
- Reduction of disease progression evidenced by cardiovascular death, hospitalization, or need to adjust heart failure medications
Pharmacology
- Non-selective antagonism of β-adrenergic receptors
- Antiproliferative properties
- Scavenger of reactive free oxidant radicals
- Used in the treatment of hypertension, angina pectoris, and congestive heart failure
Dosage & Administration
- Hypertension: initially 12.5 mg once daily, increased to 25 mg once daily, maximum 50 mg daily
- Angina pectoris: 12.5 mg twice daily for 2 days, then 25 mg twice daily, maximum 50 mg daily
- Heart failure: initially 3.125 mg twice daily, increased to 6.25 mg, then 12.5 mg, then 25 mg, maximum 25 mg twice daily for patients less than 85 kg, 50 mg twice daily for patients over 85 kg
Interaction
- with Digoxin, Rifampin, Warfarin, Clonidine
Contraindications
- Decompensated heart failure requiring intravenous inotropic therapy
- Bronchial asthma or related bronchospastic conditions
- Second or third-degree AV block
- Sick sinus syndrome
- Cardiogenic shock
- Severe bradycardia
