Description
- Dilapress is indicated for the treatment of mild, moderate, or severe heart failure of ischemic or cardiomyopathic origin, in conjunction with digitalis, diuretics, and ACE inhibitor, to reduce the progression of disease as evidenced by cardiovascular death, cardiovascular hospitalization, or the need to adjust other heart failure medications.
- Carvedilol is a cardiovascular drug whose main pharmacological action is non-selective antagonism of β-adrenergic receptors but which also possesses appreciable α-adrenergic antagonistic activity. It also has antiproliferative properties and is a scavenger of reactive free oxidant radicals. It is used in the treatment of hypertension, angina pectoris, and congestive heart failure.
- Dilapress dosage varies for hypertension, angina pectoris, and heart failure, with different initial doses and adjustments based on patient weight and condition.
- Dilapress may interact with drugs such as digoxin, rifampin, warfarin, and clonidine, affecting their levels or actions in the body.

Dosage & Administration
- Initial dose for hypertension: 12.5 mg once daily, increased to 25 mg once daily after 2 days, with a maximum of 50 mg daily in single or divided doses.
- Recommended dose for angina pectoris: 12.5 mg twice daily for the first 2 days, followed by 25 mg twice daily, with a maximum daily dose of 50 mg in divided doses for elderly patients.
- Initial dose for heart failure: 3.125 mg twice daily (with food), with gradual increments to the highest tolerated dose.
Interaction
- Increased monitoring of digoxin levels recommended when used concomitantly with Dilapress.
- Pretreatment with rifampin may reduce Cmax and AUC of Dilapress.
- Dilapress does not alter the in vitro plasma protein binding of warfarin.
- β-receptor antagonists may potentiate the pressor reaction following the sudden withdrawal of treatment with clonidine.
