Indications
- Treatment of mild, moderate, or severe heart failure
- Reduction of disease progression
- Conjunction with digitalis, diuretics, and ACE inhibitor
- Usage in patients unable to tolerate an ACE inhibitor
- Usage in patients not receiving digitalis, hydralazine, or nitrate therapy
Pharmacology
- Non-selective antagonism of β-adrenergic receptors
- A-adrenergic antagonistic activity
- Antiproliferative properties
- Scavenger of reactive free oxidant radicals
- Treatment of hypertension, angina pectoris, and congestive heart failure
Dosage & Administration
- Initial dosage in hypertension: 12.5 mg once daily
- Usual dosage in hypertension: 25 mg once daily
- Maximum dosage in hypertension: 50 mg daily
- Initial dosage in angina pectoris: 12.5 mg twice daily
- Recommended dosage in angina pectoris: 25 mg twice daily
- Maximum dosage in angina pectoris: 50 mg daily
- Initial dosage in heart failure: 3.125 mg twice daily
- Maximum dosage in heart failure: 25 mg twice daily (patients less than 85 kg)
Interaction
- Increased levels of digoxin when taken together with Koreg
- Decrease in Cmax and AUC with pretreatment of rifampin
- No alteration in the in vitro plasma protein binding of warfarin by Koreg
- β-receptor antagonists potentiate the pressor reaction following sudden withdrawal of treatment with clonidine
Contraindications
- Decompensated heart failure requiring intravenous inotropic therapy
- Bronchial asthma or related bronchospastic conditions
- Second or third-degree AV block
- Sick sinus syndrome (unless a permanent pacemaker is in place)
- Cardiogenic shock
- Severe bradycardia

- Caution in hepatic impairment
- Monitoring clinical status after initiation and increasing each dose
- Ensuring renal function and heart failure are not deteriorating
