Indications and Usage

• Treatment of hypertension alone or with other antihypertensive agents
• Lowering blood pressure
• Initial therapy in patients likely to need multiple antihypertensive agents
• Individualized therapy based on baseline blood pressure and target goal

Pharmacology

• Amlodipine as a dihydropyridine calcium channel blocker
• Amlodipine's effect on vascular smooth muscle cells and cardiac muscle cells
• Olmesartan Medoxomil as an angiotensin II receptor blocker
• Blocking the vasoconstrictor and aldosterone-secreting effects of angiotensin II

Dosage and Administration

• Substitute individually titrated components
• Initial therapy at 5/20 mg once daily
• Titration up to a maximum of 10/40 mg once daily
• Adjusted starting dose for elderly patients

Interactions

• Attenuation of antihypertensive effect by NSAIDs including selective COX-2 inhibitors
• Closely monitoring blood pressure, renal function, and electrolytes

Contraindications

• Co-administration with Aliskiren in patients with diabetes

Side Effects

• Common side effects include peripheral edema, headache, flushing, and dizziness
• Possibility of causing sprue-like enteropathy

Pregnancy and Lactation

• Pregnancy Category D
• Not for use in 2nd and 3rd trimester due to potential fetal death
• Consideration for discontinuing drug during nursing

Precautions and Warnings

• Caution for hypotension in volume- or salt-depleted patients
• Risk of vasodilation in patients with severe aortic stenosis
• Potential for increased angina or acute MI in patients with severe obstructive coronary artery disease

Use in Special Populations

• Unestablished safety and effectiveness in pediatric patients
• No overall differences in safety or effectiveness observed in subjects 65 years of age or older
• No studies in patients with renal impairment
• Initial therapy not recommended in hepatically impaired patients
