Indications

  • Treatment of moderate to severe pneumonia
  • Treatment of febrile neutropenia
  • Treatment of urinary tract infections
  • Treatment of skin and skin structure infections
  • Treatment of intra-abdominal infections
  • Indicated for cases associated with concurrent bacteremia

Pharmacology

  • Bactericidal
  • Disrupts synthesis of peptidoglycan layer of bacterial cell walls
  • Same mode of action as other beta-lactam antibiotics

Dosage & Administration

  • Administered intravenously over approximately 30 minutes
  • Specific dosage for different types of infections
  • Different dosage for moderate to severe and mild to moderate infections
  • Intramuscular route of administration indicated for specific UTIs

Interaction

  • Requires careful monitoring with high doses of aminoglycosides
  • Nephrotoxicity potential with concomitant administration of other cephalosporins with potent diuretics

Contraindications

  • Immediate hypersensitivity reactions to cefepime or cephalosporin class of antibiotics
  • Contraindicated in patients with penicillin or other betalactum antibiotics allergies

  • Potential for immediate hypersensitivity reactions

Pregnancy & Lactation

  • Category B pregnancy rating
  • Excreted in human breast milk in very low concentrations
  • Caution required when administered to nursing women

Precautions & Warnings

  • Avoid prescribing in absence of proven or strongly suspected bacterial infection
  • Prolonged use may result in overgrowth of non susceptible microorganisms
  • Associated with a fall in prothrombin activity
  • Caution required in individuals with a history of gastrointestinal disease
  • Arginine can alter glucose metabolism and elevate serum potassium

Use in Special Populations

  • Maximum dose for pediatric patients should not exceed recommended adult dose
  • Dosage adjustment necessary for patients with impaired renal function
  • No adjustment necessary for patients with impaired hepatic function

Overdose Effects

  • Careful observation and supportive treatment required
  • Hemodialysis recommended in the presence of renal insufficiency
  • Symptoms of overdose include encephalopathy, myoclonus, seizures

Reconstitution

  • Intravenous administration over a period of three to five minutes
  • Compatible with various IV infusion fluids and diluents for intramuscular route
  • Specific volumes to be added for different vial sizes
