Therapeutic Class

Indications

  • Treatment of moderate to severe pneumonia
  • Management of febrile neutropenia
  • Management of uncomplicated and complicated urinary tract infections
  • Treatment of uncomplicated skin and skin structure infections
  • Treatment of complicated intra-abdominal infections

Pharmacology

  • Bactericidal nature
  • Disruption of peptidoglycan layer synthesis in bacterial cell walls
  • Mode of action similar to other beta-lactam antibiotics

Dosage & Administration

  • Intravenous administration over approximately 30 minutes
  • Varied dosage depending on the type and severity of infection

Interaction

  • Increased potential of nephrotoxicity with high doses of aminoglycosides
  • Nephrotoxicity reported with concomitant administration of cephalosporins and potent diuretics

Contraindications

  • Hypersensitivity reactions to cephalosporin antibiotics
  • Hypersensitivity reactions to penicillin or other betalactum antibiotics

Pregnancy & Lactation

  • Pregnancy Category B
  • Excretion in human breast milk in very low concentrations
  • Caution advised during administration to nursing women

Precautions & Warnings

  • Avoiding use in the absence of a proven or strongly suspected bacterial infection
  • Risk of overgrowth of non-susceptible microorganisms with prolonged use
  • Association with a fall in prothrombin activity
  • Caution advised in individuals with a history of gastrointestinal disease
  • Alteration of glucose metabolism and transient elevation of serum potassium with arginine

Use in Special Populations

  • Maximum pediatric dose should not exceed recommended adult dose
  • Careful dose selection and monitoring in geriatric patients with renal insufficiency
  • No adjustment necessary for patients with impaired hepatic function
  • Dose adjustment required in patients with impaired renal function

Overdose Effects

  • Need for careful observation and supportive treatment
  • Recommendation of hemodialysis in the presence of renal insufficiency

Reconstitution

  • Intravenous administration compatibility with various IV infusion fluids
  • Intramuscular administration compatibility with specific diluents
  • Specific volumes of diluent to be added for different vial sizes
