Indications

  • Treatment of hypertension alone or with other antihypertensive agents
  • Lowering blood pressure
  • Initial therapy for patients needing multiple antihypertensive agents
  • Individualized blood pressure goals

Pharmacology

  • Amlodipine as a calcium channel blocker
  • Greater effect on vascular smooth muscle cells than on cardiac muscle cells
  • Peripheral arterial vasodilator
  • Olmesartan Medoxomil blocking vasoconstrictor and aldosterone-secreting effects of angiotensin II

Dosage & Administration

  • Initial therapy with 5/20 mg once daily
  • Titration up to maximum of 10/40 mg once daily
  • Lower starting dose of Amlodipine for elderly patients

Interaction

  • Attenuation of antihypertensive effect by NSAIDs including selective COX-2 inhibitors
  • Close monitoring of blood pressure, renal function, and electrolytes for patients on combination therapy

Contraindications

  • Co-administration with Aliskiren in patients with diabetes

Side Effects

  • Peripheral edema
  • Headache
  • Flushing
  • Dizziness
  • Sprue-like enteropathy

Pregnancy & Lactation

  • Pregnancy Category D
  • Contraindicated in 2nd and 3rd trimester
  • Potential adverse effects on nursing infant

Precautions & Warnings

  • Risk of hypotension in volume- or salt-depleted patients
  • Vasodilation in patients with severe aortic stenosis
  • Increased frequency, duration, or severity of angina or acute MI in patients with severe obstructive coronary artery disease

Use in Special Populations

  • Safety and effectiveness in pediatric patients not established
  • No overall differences in safety or effectiveness observed between subjects 65 years of age or older and younger subjects
  • No studies in patients with renal impairment
  • Initial therapy not recommended in hepatically impaired patients

  • No information on overdose in humans
