Indications
- Treatment of pneumonia (moderate to severe)
- Treatment of febrile neutropenia
- Treatment of uncomplicated and complicated urinary tract infections
- Treatment of uncomplicated skin and skin structure infections
- Treatment of complicated intra-abdominal infections
Pharmacology
- Cephalosporins are bactericidal
- Disrupts the synthesis of peptidoglycan layer of bacterial cell walls
- Importance for cell wall structural integrity in Gram-positive organisms
Dosage & Administration
- Administered intravenously over approximately 30 minutes
- Doses for various infections such as pneumonia, urinary tract infections, and skin infections
Interaction
- Monitor renal function when administering with aminoglycosides
- Increased potential of nephrotoxicity and ototoxicity with aminoglycoside antibiotics
- Nephrotoxicity reported with concomitant administration of other cephalosporins with potent diuretics
Contraindications
- Contraindicated in patients with immediate hypersensitivity reactions to cefepime or cephalosporin class of antibiotics
Side Effects
- Associated with immediate hypersensitivity reactions
Pregnancy & Lactation
- Pregnancy Category B
- Excreted in human breast milk in very low concentrations
- Caution advised for nursing women
Precautions & Warnings
- Avoid prescribing in the absence of proven or strongly suspected bacterial infection
- Reevaluation of patient's condition essential during prolonged use
- Associated with fall in prothrombin activity
- Prescribed with caution in individuals with a history of gastrointestinal disease
- Arginine shown to alter glucose metabolism and elevate serum potassium transiently
Use in Special Populations
- Maximum pediatric dose should not exceed the recommended adult dose
- Serious adverse events reported in geriatric patients with renal insufficiency
- No adjustment necessary for patients with impaired hepatic function
- Dose adjustment necessary in patients with impaired renal function
Overdose Effects
- Patients who receive an overdose should be carefully observed and given supportive treatment
- In the presence of renal insufficiency, hemodialysis recommended

Reconstitution
- For IV, the resulting solution should be injected directly into the vein over a period of three to five minutes
- Compatibility with various IV infusion fluids and diluents for intramuscular administration
- Specific volume of diluent to be added for different vial sizes
