Indications

Pharmacology

  • Binds to penicillin binding proteins
  • Inhibits peptidoglycan synthesis
  • Bactericidal
  • Broad spectrum
  • Active against gram-negative organisms
  • Activity against gram-positive bacteria

Dosage & Administration

  • Adult dosage: 1 gm every 12 hourly for mild to moderate infections
  • Variation of dosage based on severity, organism sensitivity, and patient's condition
  • Increased dosage for severe infections
  • Children dosage range: 100-150 mg/kg/day
  • Possible neonatal dosage: 50 mg/kg/day
  • Increased neonatal dosage for severe infections
  • Specific dosage for gonorrhea: 500 mg as a single dose

Interaction

  • Increased nephrotoxicity with concomitant administration of cephalosporins and aminoglycoside antibiotics

Contraindications

  • Hypersensitivity to cefotaxime or cephalosporin antibiotics

  • Excretion in breast milk

  • Caution in individuals with gastrointestinal disease
  • Dosage reduction in patients with renal insufficiency
  • Continued dosage adjustment in renal impairment

Use in Special Populations

  • Dosage reduction in severe renal failure
  • Further dosage adjustment based on infection course and patient's condition
