Description

  • Rolip is a HMG-CoA reductase inhibitor used to reduce elevated total-C, LDL-C, ApoB, non HDL-C and TG levels and to increase HDL-C.

Dosage and Administration

  • Usual dose range is 5-20 mg/day for pediatric patients (10 to 17 years of age)
  • Maximum recommended dose is 20 mg/day
  • 40 mg once daily for very high cholesterol and high risk of MI or stroke
  • Recommended starting dose for homozygous familial hypercholesterolemia is 20 mg once daily
  • Initiation of therapy with 5 mg once daily should be considered for Asian patients
  • Dose should be limited to 5 mg once daily with Cyclosporine
  • Dose should be limited to 10 mg once daily with Lopinavir/Ritonavir or Atazanavir/Ritonavir
  • Dose reduction should be considered when used in combination with Niacin, Fenofibrate, or Gemfibrozil
  • For severe renal impairment, dosing should be started at 5 mg once daily and not exceed 10 mg once daily

Pharmacology

  • Selective and competitive inhibitor of HMG-CoA reductase
  • Increases the number of hepatic LDL receptors
  • Inhibits hepatic synthesis of VLDL

Indications

  • Primary hyperlipidemia and mixed dyslipidemia
  • Hypertriglyceredemia
  • Primary dysbetalipoproteinemia (Type III hyperlipoproteinemia)
  • Homozygous familial hypercholesterolemia (HoFH)
  • Slowing the progression of atherosclerosis
  • Pediatric patients (10 to 17 years of age) with heterozygous familial hypercholesterolemia (HeFH)
  • Risk reduction of Myocardial Infarction (MI), stroke and arterial revascularization procedures in patients without clinically evident CHD, but with multiple risk factors

Interaction

  • Increases Rolip exposure with Cyclosporine, Lopinavir/Ritonavir or Atazanavir/Ritonavir
  • Prolongs international normalized ratio (INR) with Coumarin anticoagulants
  • Enhances risk of skeletal muscle effects with concomitant lipid-lowering therapies
  • Combination therapy with Gemfibrozil should be avoided

Contraindications

  • Known hypersensitivity to product components
  • Liver disease
  • Pregnant women and women who may become pregnant
  • Nursing mothers

Side Effects

  • Headache
  • Myalgia
  • Constipation
  • Asthenia
  • Abdominal pain
  • Nausea

Pregnancy and Lactation

  • Administered to women of childbearing age only when highly unlikely to conceive
  • If patient becomes pregnant, therapy should be discontinued immediately
  • Not known whether excreted in human milk

Precautions and Warnings

  • Prescribed with caution in patients with predisposing factors for myopathy
  • Increased risk of myopathy with concurrent administration of other lipid-lowering therapies
  • Recommended liver enzyme tests before and at 12 weeks following initiation of therapy and any elevation of dose
