Indications
- Treatment of hypertension alone or with other antihypertensive agents
- Initial therapy for patients requiring multiple antihypertensive agents
- Individualized therapy based on baseline blood pressure and target goal
Pharmacology
- Amlodipine - dihydropyridine calcium channel blocker
- Greater effect on vascular smooth muscle cells than on cardiac muscle cells
- Peripheral arterial vasodilator
- Olmesartan Medoxomil - blocks vasoconstrictor effects of angiotensin II
- Selective inhibitor of the AT1 receptor
Dosage & Administration
- Substitute individually titrated components for patients on Amlodipine and Olmesartan Medoxomil
- Initial therapy with 5/20 mg once daily, titratable up to 10/40 mg once daily
- Lower starting dose of Amlodipine for elderly patients
Interaction
- Potential attenuation of antihypertensive effect by NSAIDs
- Need for close monitoring of blood pressure, renal function, and electrolytes
Contraindications
- Contraindicated with co-administration of Aliskiren in diabetic patients
Side Effects
- Common side effects include peripheral edema, headache, flushing, dizziness
- Potential for intestinal problems (sprue-like enteropathy)
Pregnancy & Lactation
- Pregnancy Category D
- Avoid use in 2nd and 3rd trimester due to risk of fetal death
- Unknown excretion in human milk, potential adverse effects on nursing infants
Precautions & Warnings
- Use with caution in volume- or salt-depleted patients
- Risk of vasodilation in severe aortic stenosis patients
- Potential for increased angina frequency, duration, or severity in severe obstructive coronary artery disease patients
Use in Special Populations
- Safety and effectiveness not established in pediatric patients
- No overall differences in safety or effectiveness in subjects 65 years or older
- No studies in patients with renal impairment
- Initial therapy not recommended in hepatically impaired patients
