Indications
- Management of hypertension
- Adjunctive therapy in edema associated with congestive heart failure
- Adjunctive therapy in edema associated with hepatic cirrhosis
- Adjunctive therapy in edema associated with corticosteroid and estrogen therapy
Pharmacology
- Chlorthalidone is a thiazide-type diuretic-antihypertensive
- Used for the treatment of hypertension
- Inhibits the reabsorption of sodium and chloride in the distal renal tubule
- Promotes water loss
- Causes potassium and sodium loss
- Long elimination half-life
- Volume reduction for blood pressure lowering effects
- Caution in patients with impaired hepatic function or progressive liver disease
- Cautious dosage schedule in patients with severe coronary or cerebral atherosclerosis
Dosage & Administration
- Therapy should be initiated with the lowest possible dose
- Titration for maximum therapeutic benefit with minimum side effects
- Usual adult dose is 25 to 50 mg daily for hypertension
- Usual adult dose should not exceed 50 mg per day for edema of specific origin
- Suitable for treating hypertension in the elderly
- Special caution in patients with renal disease or impaired hepatic function
Interaction
- Caution with impaired hepatic function
- Enhanced antihypertensive effects in postsympathectomy patients
- Potentiation with antihypertensive agents
- Potential increase in digitalis-induced cardiac arrhythmias
- Increased hypokalemic effects with corticosteroids
- Possible weakening of diuretic and antihypertensive activity with NSAIDs
- Increased responsiveness to curare derivatives and ganglionic blocking agents
- Potential hypersensitivity reactions with allopurinol
- Increased risk of adverse effects from amantadine
- Reduced renal excretion of cytotoxic agents with antineoplastic agents
- Bioavailability increase with anticholinergics
- Decreased absorption by cholestyramine
- Decreased urinary excretion of calcium with thiazide-type diuretics and Vitamin D
- Risk of hyperuricemia and gout-type complications with cyclosporin
- Increased calcium reabsorption with calcium salts
- Enhanced hyperglycemic effect of diazoxide
Contraindications
- Anuria
- Severe renal failure
- Severe hepatic failure
- Refractory hypokalemia or conditions involving enhanced potassium loss
- Hyponatremia
- Hypercalcemia
- Symptomatic hyperuricemia
- Hypersensitivity to Chlorthalidone and other sulfonamide derivatives or their excipients
- Caution in patients with renal disease or impaired hepatic function
- Periodic determination of BUN and serum creatinine in patients with renal disease
- Possible loss of diuretic efficacy with GFR below 30 mL/min
- Monitoring potassium balance in various conditions
Side Effects
- Frequent: hypokalemia, hyperuricemia and rise in blood lipids
- Occasional: hyponatremia, hypomagnesemia and hyperglycemia
- Rare: hypercalcemia, glycosuria, worsening of diabetic metabolic state and gout
- Isolated cases: hypochloremic alkalosis
- Dermatology: urticaria, skin rash, photosensitization
- Liver: intrahepatic cholestasis, jaundice
- Cardiovascular: postural hypotension, cardiac arrhythmias
- CNS: dizziness, slow mentation, decreased reaction time
Pregnancy & Lactation
- Avoid use in pregnancy especially in the first trimester
- Avoid use in nursing mothers due to passage into breast milk
Precautions & Warnings
- Thalin dosage reduction in moderate renal failure
- Avoid use in advanced renal failure
- Risk of precipitating hepatic encephalopathy in patients with liver cirrhosis and ascites
- Avoid use in pregnancy
- Avoid use in lactating mothers
- Suppression of lactation in lactating mothers
Overdose Effects
- Symptoms include nausea, weakness, dizziness, disturbances of electrolyte balance
- No specific antidote
- Recommend gastric lavage followed by supportive treatment
