Description
Chelova is indicated for the treatment of chronic iron overload due to repeated blood transfusions in adult and pediatric patients aged 2 years and over with thalassaemia, sickle cell disease, and myelodysplastic syndrome. Also indicated for treating chronic iron overload in patients with non-transfusion-dependent thalassemia syndromes aged 10 years and older.
Indications

Pharmacology
- Orally active chelator
- Highly selective for iron
- Binds iron with high affinity in a 2:1 ratio
- Promotes excretion of iron primarily in the faeces
- Low affinity for zinc and copper
- Induces mean net excretion of iron in iron overloaded patients
Dosage
- Recommended initial daily dose of 20-40 mg/kg body weight
- Should be taken on an empty stomach
- Requires monitoring of serum ferritin levels
- Dose adjustment based on serum ferritin trends
Administration
- Taken once daily on an empty stomach
- Tablets dispersed in water or juice for swallowing
- Not to be chewed or swallowed whole
- Dispersion in carbonated drinks or milk not recommended
Interaction
- Should not be taken with aluminium-containing antacids
- Cautious use with drugs metabolized through CYP3A4
- Consider dose increase with certain concomitant drugs
- Caution with drugs having ulcerogenic potential
- Interaction with other CYP2C8 substrates cannot be excluded
Contraindications
- Hypersensitivity to the active substance or excipients
- Creatinine clearance less than 40 ml/min
- Serum creatinine >2 times the upper limit of normal
Side Effects
- Cytopenias
- Gastrointestinal issues like diarrhea, constipation, and abdominal pain
- Headache and dizziness
- Elevation of hepatic transaminases
- Cases of Stevens-Johnson syndrome and skin rash reported
- Auditory and ocular disturbances
Pregnancy & Lactation
- Pregnancy Category C
- No adequate and well-controlled studies in pregnant women
- Use during pregnancy only if benefits justify potential risks
- Not known whether excreted in human milk
Precautions & Warnings
- Risk of gastrointestinal bleeding, ulceration, and irritation
- Cytopenias including agranulocytosis and thrombocytopenia
- Serious hypersensitivity reactions reported
Use in Special Populations
- Pediatric use similar to adult use
- Elderly patients at increased risk for toxicity
- Not studied in patients with renal or hepatic impairment
Overdose Effects
- Maximal dose of 40 mg/kg body weight
