Pharmacology

Dosage & Administration
- 300 mg oral loading dose for acute coronary syndrome
- 75 mg daily for recent MI, stroke, peripheral arterial disease
- Oral administration with or without food
Interaction
- Increased risk of bleeding with NSAIDs, warfarin, SSRIs, SNRIs
- Avoid concomitant use with CYP2C19 inhibitors
- Increased plasma concentrations with Repaglinide concomitant use
Contraindications
- Hypersensitivity to the drug substance
- Active pathological bleeding
Side Effects
- Common side effects: Bleeding, Diarrhoea, gastrointestinal discomfort, haemorrhage, Skin reactions
- Rare side effects: Acquired haemophilia, anaemia, angioedema, arthralgia, arthritis, bone marrow disorders
Pregnancy & Lactation
- Use during pregnancy only if clearly needed
- Unknown excretion in human breast milk
- Consideration for discontinuing nursing or drug during breastfeeding
Precautions & Warnings
- Impaired metabolism by genetic variations in CYP2C19
- Increased risk of bleeding due to lifetime platelet aggregation inhibition
- Discontinuation risk for cardiovascular events
- Reported Thrombotic Thrombocytopenic Purpura (TTP)
- Hypersensitivity reactions
Use in Special Populations
- Safety and effectiveness not established in pediatric populations
- No dosage adjustment in elderly patients
