Indications
- Treatment of hypertension
- May be used alone or in combination with other antihypertensive agents
Pharmacology
- Formation of angiotensin II
- Vasoconstrictor effects of angiotensin II
- Blocking the binding of angiotensin II to the AT 1 receptor
- Principal pressor agent of the renin-angiotensin system
- Stimulation of synthesis and release of aldosterone
- Cardiac stimulation
- Renal reabsorption of sodium
Dosage & Administration
- Usual recommended starting dose of 20 mg once daily
- May be increased to 40 mg for further reduction in blood pressure
- No advantage of twice-daily dosing over once daily
- No initial dosage adjustment recommended for elderly patients
- Consideration for lower starting dose in patients with possible intravascular volume depletion
- May be administered with or without food
- Safety and effectiveness in pediatric patients not established
Interaction
- No significant drug interactions reported with digoxin or warfarin
- Bioavailability not significantly altered by antacids
- Not metabolized by the cytochrome P450 system
- Effects on P450 enzymes not expected
- Potential deterioration of renal function with NSAIDs
- Attenuation of antihypertensive effect by NSAIDs
Contraindications
- Hypersensitivity to any component of the product
Side Effects
- Well tolerated with similar incidence of adverse reactions to placebo
- Adverse reactions not dose related
- Common adverse reactions include dizziness, back pain, diarrhea, headache, hyperglycemia, and cough
Pregnancy & Lactation
- Discontinue use when pregnancy is detected
- Can cause injury to developing fetus in second and third trimesters
- Decision should be made whether to discontinue nursing or discontinue the drug
Precautions & Warnings
- Changes in renal function may be anticipated
- Association with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals
Overdose Effects
- No experience of overdose with the product
- Likely effects of hypotension and tachycardia
- Gastric lavage or induction of emesis may be considered if overdose is recent
- Clinically significant hypotension requires active support of the cardiovascular system
