Indications

  • Osteoarthritis
  • Rheumatoid arthritis
  • Ankylosing spondylitis
  • Decreasing the risk of gastric ulcers

Pharmacology

  • Immediate release Esomeprazole Magnesium layer
  • Enteric-coated Naproxen core
  • Mechanism of action to inhibit prostaglandin synthesis
  • Proton pump inhibitor to suppress gastric acid secretion

Dosage

  • Consideration of potential benefits & risks
  • Use of lowest effective dose
  • Different doses for adults and adolescent patients
  • Administration at least 30 minutes before meals

Administration

  • Not to split, chew, crush or dissolve the tablet

Interaction

  • Reduction of antihypertensive effect with concomitant use of NSAIDs
  • Increased risk of bleeding complication with warfarin
  • Interference with the absorption of certain drugs by Esomeprazole
  • Effect of high-fat food on absorption of Naproxen & Esomeprazole
  • Prolonged tmax and decreased Cmax with high-fat food

Contraindications

  • Hypersensitivity to tablet components
  • History of asthmatic or allergic-type reactions to aspirin or other NSAIDs
  • Use during peri-operative period in coronary artery bypass graft (CABG) surgery

Side Effects

  • Decrease in gastrointestinal side effects with immediate release esomeprazole
  • Lower risk of arterial thrombotic events with Naproxen
  • Adverse events including oedema, hypertension, cardiac failure
  • Gastrointestinal adverse events and symptoms

Pregnancy & Lactation

  • Use caution during first and second trimester of pregnancy
  • Contraindicated during third trimester of pregnancy
  • Excretion of naproxen in low quantities in human milk
  • Unknown excretion of esomeprazole in human milk
  • Not recommended for women attempting to conceive

Precautions & Warnings

  • Avoidance of NSAIDs including cyclooxygenase-2 selective inhibitors
  • Use with low dose acetylsalicylic acid
  • Minimization of undesirable effects by using lowest effective dose for shortest duration
  • Risk-factors for NSAID related gastro-intestinal complications
  • Surveillance for patients on long-term treatment
  • Increased frequency of adverse reactions in older people
  • Gastrointestinal, cardiovascular, cerebrovascular, and renal effects

Use in Special Populations

  • Increased unbound plasma fraction of naproxen in elderly patients
  • Not recommended in patients with moderate to severe renal impairment
  • Close monitoring in patients with mild to moderate hepatic impairment
  • Consideration of dose reduction in severe hepatic impairment

Overdose Effects

  • Symptoms related to Naproxen and esomeprazole overdose
  • Management of overdose with symptomatic and supportive care
  • No specific antidotes for naproxen or esomeprazole overdose
