Indications
- Adjunct to diet and exercise in type 2 diabetes mellitus patients
- Monotherapy or replacement of combination therapy of glimepiride and metformin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates release of insulin
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Decreases intestinal absorption of glucose
Dosage
- Governed by desired blood glucose level
- Lowest dosage sufficient for metabolic control
- Regular measurement of glucose levels in blood and urine
- Maximum recommended daily doses
- Starting dose not to exceed existing doses of glimepiride or metformin
Administration
Must be swallowed whole and not crushed or chewed
Interaction
- Glimepiride:
- Metabolized by cytochrome P450 2C9 (CYP2C9)
- Potentiation and weakening of blood-glucose-lowering effect with various drugs
- Exhibits unpredictable interaction with alcohol
- Metformin:
- Concomitant use not recommended with alcohol
- Discontinue prior to imaging procedure with iodinated contrast agents
- Requires precautions when used in combination with certain medicinal products
- Substrate of organic cation transporters (OCT)
Contraindications
- Glimepiride:
- Hypersensitivity to glimepiride or metformin
- Pregnant or breastfeeding women
- Metformin:
- Hypersensitivity to metformin
- Acute metabolic acidosis, severe renal failure, hepatic insufficiency, alcohol intoxication, lactation

- Changes in blood picture
- Hepatitis and liver enzyme elevation
- Allergic reactions
- Weight gain
- Metformin:
- Gastrointestinal symptoms

- Vitamin B12 absorption decrease
- Lactic acidosis
- Hemolytic anemia
- Reduction of thyrotropin level
Pregnancy & Lactation
- Glimepiride: Not recommended during pregnancy or breastfeeding
- Metformin: Avoid use during pregnancy, caution in lactation
Precautions & Warnings
- Glimepiride: Increased risk of hypoglycemia in initial weeks of treatment, careful monitoring required
- Metformin: Regular monitoring of thyroid-stimulating hormone (TSH) levels, potential decrease in vitamin B12 levels
Use in Special Populations
- Children: Insufficient data for pediatric use
- Renal impairment:
- Assessment of glomerular filtration rate (GFR) before initiation
- Dose adjustments based on GFR levels
Overdose Effects
- Glimepiride: May lead to severe life-threatening hypoglycemia, requires immediate medical attention
- Metformin: May lead to lactic acidosis, a medical emergency
